Our care managers listen to you. We help you identify and understand your health care needs, as well as care and treatment options to address those needs. Your care management team includes registered nurses and social worker care managers with support from pharmacists, physical therapists and other health care professionals who collaborate to find options and services that promote quality and effective interventions and outcomes for you. You will speak with a registered nurse, social worker or health and wellness coach.
